Welcome to Dr. Ralph Martin's Web site. We hope that we can be of some assistance to you and would like to describe some of the unique features of Dr. Martin's medical practice.
Before
Dr. Martin will accept you as his patient, you must undergo a
complete history and physical examination. This examination is
quite extensive and will establish a baseline for your therapy.
Your
Medical Records are secure with Dr. Martin. He will not share
them with your employer or health maintenance organization
without your explicit permission.
Dr.
Martin works for you alone. He is not beholden to any insurance
plan or government program. Because of this focus, you can be
sure that your interests remain his utmost priority.
Dr.
Martin has a very busy practice. He does not need to do
"unnecessary tests" or procedures. In addition if Dr.
Martin feels that he can not help you, he will quickly refer you
to another doctor.
Before
you are seen by Dr. Martin, you will need to complete a Personal
History Form. A staff member will mail this to you when you make
an initial appointment with Dr. Martin. Please bring this with
you, completed, on your first visit.
You may open and print our Patient Registration Form. Please bring this with you completed on your first visit.
Please
obtain your past medical information as soon as possible. Please
bring these records in with you, or have them sent to Dr. Martin.
