INSURANCE INFORMATION
Dr. Martin is not contracted with any insurance company. When a physician is contracted with an insurance company the physician must have tests and procedures authorized. On many occasions insurance companies will deny authorization due to the feeling that some tests are done preventatively.
A charge ticket is given to the patient when they have an appointment. Information provided on the charge are procedure codes and diagnoses codes which are required to bill a patient's insurance. A copy of this 2-part form should be attached to the patient's insurance form. The patient should submit this to their insurance company for reimbursement.
We quite often receive correspondence from patient's insurance companies requesting information on the services provided. They may need a clarified procedure number or diagnosis code. We make an effort to answer these requests as soon as possible, so that the patient can get reimbursed.
Dr. Martin does practice "Preventative Medicine" He strongly feels that performing different tests and procedures that are not "Medically Necessary" quite often discover underlying medical problems that can be addressed early. Unfortunately, most insurance companies will not cover these types of procedures.
