New Patient Letter

  Dear Patient,

Dr. Ralph Martin and his staff welcome you and thank you for your interest in having Dr. Martin to be your physician. We sincerely hope that your first visit and all subsequent visits will be a pleasant experience. Because our patients are important to us, we believe that you should always be aware and informed of all matters relating to your health. To accomplish this, you have been scheduled for a complete personal history and physical examination.

The first step is to fill out the enclosed personal history form as completely as possible. Even the smallest bit of information will be helpful in evaluation of your case. Please bring the completed form with you the day of your exam.

Secondly, so that you may arrange your schedule accordingly, we generally ask that you allow approximately 2 to 2 1/2 hours for your exam to be completed.

On the day of your appointment, Dr. Martin will evaluate your history form and discuss with you any problems that you are currently having. You will then have your physical examination and laboratory blood work. Due to the nature of the testing, you will need to have been fasting 14 hours prior to your appointment and also not consume any alcohol for 24-hours. You should drink water, and please continue to take your regular medications. A return visit will be scheduled to discuss all of the findings with Dr. Martin approximately two weeks following your physical.

The cost of the examination will be approximately $1500. This includes all lab work and x-ray (if needed). The laboratory and radiology will bill you separately. Our office fees are expected to be paid at the time of your visit. In order to keep your billing costs to a minimum, our office does not bill insurance. We will however provide the patient with a "Superbill" that is ready for the patient to submit to their insurance for reimbursement. Dr. Martin strongly feels that practicing "preventative medicine" is important in finding a variety of diseases in their early stages. Unfortunately, most insurance companies do not feel or practice this way, and will not reimburse for "preventative medicine". We will be happy to answer any questions regarding insurance matters.

As your physician, Dr. Martin asks to see you yearly for a complete physical.
